6. BRIEF RESUME OF THE INTENDED WORK:
6.1 Need for study-

Kikkisa is a Tridoshaj Vyadhi, clinically characterized by burning, itching and stretch marks on the body particularly on the abdomen, thighs and breast region. It is usually found in the second trimester of pregnancy particularly in the 7th month due to the overstretching of the abdominal skin. It can be correlated with Striae Gravidarum from modern science.

Striae Gravidarum is the major problem of modern cosmetic era, here more than 90 % of the woman suffers from this condition. It makes much psychological distress to the woman. They are clinically by linear bands that are initially erythematous and gradually fade to become skin coloured or hypo pigmented atrophic lines that may be thin or wide.

Many woman use moisturizening creams and lotions during their pregnancy period to avoid stretch marks and worry about them constantly. But most of these creams only provide temporary moisture that last for few hours and thus are not enough to prevent the stretch marks. Many other preparations are also there but they are not that much effective and can cause allergic reaction in some peoples and cost effective is also one of the problem.

In Ayurveda many preparations are mentioned for the Kikkisa Vyadhi like Pana, Lepa, Abhyanga, Parisheka which are very easy to prepare, safe to use, and without any side effects and also cost effective. Hence an attempt will be made in present study to evaluate the efficacy of Chandana-Ushiradi Kalka (Astang Samgraha Sharir 3/4) and Nimb-Koladi Kalka (Charaka samhita Sharir 8/31) in the management of Kikkisa.

6.2 Review of Literature:


  1. Description of Kikkisa Nidan , Laxanas and Chikitsa as per the authentic text books of Ayurveda.

  2. Description of Striae Gravidarum according to modern science.

  3. Description of Chandana-Ushiradi kalka and Nimba-Koladidi kalka in Ayurvedic literature.



Previous work done:

  1. Dr. Devokate M. B. – Garbhini Kikkisa ek sarwangin abhyasa – in 1997 from Pune.

  2. Dr. Hawalw Anmol A. – To study the role of Karveer patra sidha taila on Kikkisa – in 2004 from Pune.

  3. Dr. Rana S. S. – A clinical study of Kikkisa & its management with Kikkisahara Ghrita & Kikkisahara lepa w.s.r. of striae gravidarum – in 2005 from Jamnagar.

  4. Dr. Gaytri Patil – A comparative study of Karveer patra sidha taila & Karanj patra sidha taila – in Karnataka.


6.3 Objectives of the study-

1. Conceptual study of Kikkisa.

2. To evaluate the efficacy of Chandanaushiradi Kalka & Nimkoladi Kalka in the

management of Kikkisa.

3. To compare the efficacy of Chandana-Ushiradi and Nimba-Koladi Kalka.

7.0 Materials and Methods-
7.1 Source of data-
a) Literary source:

The details of literature of Kikkisa and Striae Gravidarum will be incorporated in great detail from samhitas and other Ayurvedic and modern reference books.


b) Clinical source:

The patient attending the OPD, IPD and by conducting special camps in

Shri. J.G.C.H. Ayurvedic Medical college and Hospital, Ghataprabha will be selected on

the basis of symptoms.


c) Drug source:

The contents of Chandana-Ushiradi Kalka such as Chandan and Ushira and contents of Nimba-Koladi Kalka such as Nimba, Kola, Surasa, Manjistha will be collected from near by market after their proper identification.


7.2 Methods of collections of data-
It is clinical study where 30 patients in each group will be selected. The signs and symptoms will be recorded on the proforma designed for the study.
A) Study Design (INTERVENTIONS) -

On the basis of inclusion criteria the screened patient of Kikkisa will be randomly classified in to two groups. Group-A and Group-B.


Group-A -

Chandana-Ushiradi kalka will be applied locally two times for two months.



Group-B-

Nimba-Koladi kalka will be applied locally two times for two months.


Follow Up-

The patients of both group will be followed up once in 15 day for next 2 months.


B) Selection Criteria -
Inclusion Criteria-

  1. 2nd and 3rd trimester of primigravida.

  2. Age 18-35 years.

  3. The patient having Striae Gravidarum over abdomen & breast are selected.

Exclusion criteria-

  1. All types of skin diseases.

  2. HIV, HbsAg, Uncontrolled diabetes will be excluded.

  3. Twins pregnancy.

  4. Polyhydroamniosis.

  5. Patients having systemic and local infection and severe systemic diseases.

  6. Malignancy of the affecting part.

  7. Pre- Eclampcia

  8. Epilepsy

C) Assessment Criteria-

Assessment will be done based on subjective and objective criteria before and after the treatment.



  1. Subjective Criteria-

  • Itching ( Kandu) (Mild/Moderate/Severe)

  • Burning (Vidah) (Mild/Moderate/Severe)

  • Discoloration (Vivarnata)



  1. Objective Criteria –

  • Width of the lesion.

  • Length of the lesion.

  • Area of lesion

  • Relief the sign and symptoms according to profarma.

  • Surface area measured in percentage before and after treatment.

  • Photograph of the involved skin were take before and after treatment as per need of the study.

Investigations:

  • Haemogram,

  • Urine routine microscopic,

Bile salt,

Bile pigments.

The data collected will be subjected to statistical analysis that is unpaired ‘t’ test for its significance.

7.3 Does this study required any investigation or intervention to be conducted on patients or other humans or animal? If so describe briefly.

Yes the study will be done on 2nd & 3rd trimester of pregnancy only externally and the required investigations and interventions are as mentioned above.



7.4 Has ethical clearance been obtained from in case of 7.3?

Yes, clearance has been obtained through the ethical committee formulated in the institution.
